Assures quality care for all clients in the 7 County area by assisting in providing  services which allow them to remain in their home with safety and dignity.

• Develop and direct plan of care for each client as well as home visits per policy
• Schedule visits and follow ups with all new and potential clients
• Monitor client satisfaction
• Assist with scheduling of clients
• Communicate with Discharge Planners, families, physicians, and other agencies in regards to client care
• Provide adequate supervision and training of new and existing staff
• Rotating evening, holiday, and weekend on-call for clients and staff

Qualifications:

• Bachelor’s degree preferred in related degree
• RN/LPN with current licensure and 2+ years of  work experience will substitute for degree specifications
• Valid DL
• Ability to pass all required background checks
• Great communication and organizational skills
• Ability to work independently
