Assures quality care for all clients in the 7 County area by assisting in providing services which allow them to remain in their home with safety and dignity.
- Develop and direct plan of care for each client as well as home visits per policy
- Schedule visits and follow ups with all new and potential clients
- Monitor client satisfaction
- Assist with scheduling of clients
- Communicate with Discharge Planners, families, physicians, and other agencies in regards to client care
- Provide adequate supervision and training of new and existing staff
- Rotating evening, holiday, and weekend on-call for clients and staff
Qualifications:
- Bachelor’s degree preferred in related degree
- RN/LPN with current licensure and 2+ years of work experience will substitute for degree specifications
- Valid DL
- Ability to pass all required background checks
- Great communication and organizational skills
- Ability to work independently
