Position Summary:
Provides oversight for the planning, implementation, and evaluation of the DSMES program. Ensures systematic and coordinated delivery of diabetes education services. Provides individualized DSMES to individual patients according to Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators and ADCES accreditation requirements.
Duties and Responsibilities:
· Leads the selection, development and ongoing review of the curriculum and educational materials to ensure they meet the needs of the target population
· Develops and directs the implementation of an annual program evaluation plan and performance improvement activities, including CQI projects
· Ensures the DSMES program accreditation requirements are met and maintained
· Arranges and coordinates activities of the Stakeholder Group
· Manages multi-departmental collaboration to coordinate scheduling, reimbursement, marketing and other program administrative functions, as needed
· Coordinates diabetes education and/or Medical Nutrition Therapy (MNT) (for RD only) consultation sessions, in-person or virtually
· Collects program participant assessment data and provides educational interventions utilizing a variety of training and counseling methods appropriate for patients based on assessment data
· Collaboratively develops health plan with patient to include education goals, desired outcomes, and follow-up plan for each program participant
· Provides patient and family support between sessions and on an ongoing basis to ensure diabetes management success
· Documents patient data in education record
· Utilizes healthcare team approach to communicate DSMES interventions and patient progress in electronic health record
· Makes recommendations to referring providers for changes in participant’s diabetes care plan as appropriate
· Works closely with CBH Physicians to promote the program to various practices in the surrounding area to improve overall community health and patient outcomes and to increase program referrals
· May conduct in-house compliance and training continuing education of staff
· Acts as a diabetes resource for physicians and staff members
· Maintains 15 hours of approved continuing education annually specific to diabetes, diabetes related topics, behavior change and/or self-management education strategies
Knowledge, Skills, and Abilities:
· Possesses managerial abilities, including but not limited to organization and attention to detail
· Working knowledge of current diabetes treatment management and educational processes and program and regulatory requirements
· Ability to lead and effectively facilitate groups
· Excellent communication and listening skills
· Basic computer skills (internet, email, web conferencing)
Education/Experience:
· Registered Dietitian or Registered Nurse (license required) who is also a Certified Diabetes Care and Education Specialist (CDCES) or who is eligible to become a CDCES at time of hire
· Bachelors of Science Degree (Preferred)
· Two + years experience preferred
Requirements:
Full-time (preferred), Part-Time or Contract.
Working Hours=Monday-Friday
